I (the student test-taker) agree to the following responsibilities:
1. I will print a copy of my completed test, as instructed on the test page, before submitting the test. This copy will allow me to re-submit the test if the test is not received by the CE provider.
2. I will contact the CE provider if I do not receive test results within 48 hrs of test submission. Any planned delays in test correcting will be noted on the home page. Please notify the provider by phone at (608)219-4985, if you do not receive test results within 48 hours.
3. I understand that the responsibility to complete the electronic affidavit is my own, and that the CE provider has up to ten days after receipt of the affidavit to submit earned credits to the state.
4. I understand that I am responsible for the renewal of my Arkansas insurance license at www.nipr.com. Receipt of CE credits does not renew my license.
5. I will not hold the CE provider (CEArkansas.net) liable for any damages I incur due to my failure to meet any of the obligations mentioned on this page or any others that I reasonably could have assumed were mine based on the insurance laws of the state of Arkansas.
6. I realize and accept that attempting to complete my CE credits in the final weeks or days of my reporting period is not recommended by the AR AID, and could result in the cancellation of my license if all the necessary testing and affidavits are not received within the legally allotted time period.
7. CEArkansas.net liability for damages incurred by student due to late reporting of test results (>10 days from date of receipt of properly filled-out state affidavit form) are limited to the testing fees paid to CEArkansas.net by student.
8. Refunds will only be made in the cases of:
a. Customer error: customer mistakenly enrolled (paid) more than once for the same course(s).
b. Provider error: credits not being reported by me to the AR AID within 10 days of receipt of your
